Overview

Cerebrolysin dosing is defined by clinical trials and product labeling in the countries where it is registered — not by community convention. The best-documented regimen is the stroke-trial pattern: 30 mL per day by slow intravenous infusion, daily for 10 to 21 days, started within days of the injury and given alongside rehabilitation. Doses are measured in milliliters of a ready-made solution, chosen by the treating physician for the indication — a fundamentally different model from the milligram-and-syringe arithmetic of research peptides.

What the stroke trials used

  • CARS (2016): 30 mL once daily by IV infusion for 21 days, started 24–72 hours after stroke, combined with standardized rehabilitation (Stroke 2016); the CARS-1/CARS-2 pooled analysis used the same protocol across 442 patients (Neurological Sciences 2017).
  • Across the wider RCT literature, schedules cluster around 30 mL daily for 10–21 days; the Cochrane review specifically breaks out results for the 30 mL/10-day schedule (Cochrane 2023).
  • Lower volumes (commonly 5–10 mL) appear in dementia studies and registered-market labeling for non-acute indications, typically given intramuscularly or by short infusion.

Courses, not continuous use

Everywhere it is studied or registered, Cerebrolysin is a course drug: a block of consecutive daily doses (often 10–20 treatment days), sometimes repeated after an interval at a physician's discretion — not an indefinite daily habit. The course structure follows the therapeutic idea: support the brain's repair processes during a recovery window after injury. That logic also explains why trials start treatment within days of a stroke; nothing in the dosing literature addresses open-ended use in uninjured people, because that was never the question being asked. The same course structure appears in the traumatic brain injury and dementia literature, with the dose volume — not the schedule concept — being what varies by indication.

How it is administered

Cerebrolysin comes as a ready-to-use sterile solution in ampoules — there is no reconstitution step. In trials and clinical practice, larger doses (10 mL and up) are diluted into an infusion bag and run in slowly over roughly 15 minutes or more, because rapid administration provokes flushing, heat sensations, and dizziness; small doses can be given intramuscularly. Slow rate, sterile handling, and someone watching for reactions are part of the dosing regimen, not optional extras — the infusion setting is where the product's tolerability record was earned.

Why this doesn't translate to self-injection

The vials sold by research-peptide vendors invite a category error: treating a physician-administered infusion biologic as if it were a subcutaneous peptide. The studied doses (30 mL/day) are not practical to self-inject, the small "equivalent" amounts people improvise have no evidence behind them, and the evidence for the real regimen is itself contested — Cochrane found no demonstrated benefit in acute stroke and a possible increase in serious adverse events (Cochrane 2023).
