Dulaglutide is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist developed as an approved prescription medicine. It is marketed under the brand name Trulicity and is designed for once-weekly subcutaneous administration. Structurally, it links a modified GLP-1 analog to a fragment of a human antibody, which is what gives it an extended duration of action compared with the body's own short-lived GLP-1.

It is important to distinguish the approved, regulated product from "research" or "for research use only" versions of dulaglutide sold outside pharmacy channels. Such vendor material is not the approved medicine, is not subject to the same quality controls, and should not be treated as interchangeable with a prescribed product.

Mechanism of action

Dulaglutide activates the GLP-1 receptor, mimicking the action of the incretin hormone GLP-1. Mechanisms described in the literature include:

  • Enhancing glucose-dependent insulin secretion from pancreatic beta cells
  • Suppressing inappropriate glucagon release when blood glucose is elevated

Because insulin secretion is glucose-dependent, the mechanism is associated with a comparatively lower intrinsic risk of hypoglycemia than some other glucose- lowering approaches, though risk rises when it is combined with agents such as insulin or sulfonylureas.

Indications and use context

In regulated medical practice, dulaglutide has been approved (depending on jurisdiction) as an adjunct to diet and exercise to improve glycemic control in adults, and in some regions adolescents, with type 2 diabetes. It also carries data relevant to cardiovascular risk reduction in certain patient populations, a point that distinguishes it within the GLP-1 class.

It is not the same product as semaglutide or liraglutide; each is a distinct molecule with its own dosing schedule, pharmacokinetics, and approval history. Any real-world use should be guided by a qualified clinician and aligned with local regulations and product labeling.

Anti-doping status

WADA context

Status: Not prohibited. Dulaglutide is not on the WADA Prohibited List, in or out of competition — but the GLP-1 class is now under active surveillance.

Dulaglutide is not named anywhere on the WADA Prohibited List, and there is no GLP-1 receptor agonist class entry. The S0 catch-all does not reach it either, since S0 applies to substances with "no current approval by any governmental regulatory health authority for human therapeutic use," and dulaglutide is an approved medicine.

What has changed is adjacent to the List rather than on it. Under Article 4.5 of the World Anti-Doping Code, WADA publishes a Monitoring Program covering "substances which are not on the Prohibited List, but which WADA wishes to monitor in order to detect potential patterns of misuse in sport." Item 6 of the 2026 Monitoring Program is "Markers of Semaglutide and Tirzepatide," in and out of competition. Dulaglutide is not named, and monitoring cannot produce an anti-doping rule violation — but the incretin class is now formally being watched, which is ordinarily the step that precedes any listing decision.

Athletes in weight-category or body-composition-judged sports should also note that federation-level rules on weight manipulation are separate from the Prohibited List and are not resolved by a substance being unlisted.

Product labeling for this class carries a boxed warning regarding thyroid C-cell tumors observed in rodent studies; the relevance to humans is uncertain, and the class is generally described as contraindicated in people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2. Pancreatitis, gallbladder-related events, and injection-site reactions are also discussed. Risk–benefit decisions, contraindications, and monitoring should always be set by a prescribing clinician.

Pharmacology and dosing considerations

Dulaglutide's antibody-fragment design gives it a long half-life that supports once-weekly administration. Clinically, the approved product follows a structured titration approach intended to improve gastrointestinal tolerability over time.

Frequently asked questions

How is dulaglutide different from semaglutide and liraglutide? All three activate the GLP-1 receptor, but they are separate molecules with different structures and dosing rhythms. Dulaglutide links a GLP-1 analog to a human antibody Fc fragment and has an elimination half-life of about 5 days, which is why the approved product is injected once weekly (Trulicity prescribing information); liraglutide instead binds albumin via a fatty-acid chain and has a half-life near 13 hours, so it is dosed daily. In head-to-head trials, once-weekly dulaglutide 1.5 mg was non-inferior to once-daily liraglutide 1.8 mg for HbA1c at 26 weeks (AWARD-6, Lancet 2014), while semaglutide 1.0 mg lowered HbA1c and weight more than dulaglutide 1.5 mg at 40 weeks (SUSTAIN 7, Lancet Diabetes Endocrinol 2018).

Why is dulaglutide given once a week instead of daily? Native GLP-1 is broken down within minutes by DPP-4, so an unmodified analog would need constant redosing. Dulaglutide's antibody-fragment scaffold makes the molecule large enough to slow clearance and resist that degradation, producing an elimination half-life of roughly 5 days per the approved labeling. The weekly schedule is a consequence of that pharmacokinetic design rather than a convenience choice layered on top of it.

What is Trulicity approved for? The US label lists two indications: as an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients 10 years of age and older with type 2 diabetes, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors (Trulicity prescribing information). Weight loss is not an approved indication for this product, which is a meaningful distinction within the GLP-1 class.

What did the REWIND cardiovascular outcomes trial find? REWIND randomized 9,901 people with type 2 diabetes to weekly dulaglutide 1.5 mg or placebo and followed them for a median of 5.4 years. The primary composite of non-fatal myocardial infarction, non-fatal stroke, or cardiovascular death occurred in 12.0% on dulaglutide versus 13.4% on placebo (hazard ratio 0.88, 95% CI 0.79–0.99; p=0.026), while all-cause mortality did not differ significantly between groups (HR 0.90, 95% CI 0.80–1.01) (Gerstein et al., Lancet 2019). Unlike several earlier GLP-1 outcome trials, REWIND enrolled participants with cardiovascular risk factors as well as those with previous cardiovascular events.

How common are the gastrointestinal side effects? They are the dominant tolerability issue. In REWIND, 47.4% of participants on dulaglutide reported a gastrointestinal adverse event during follow-up versus 34.1% on placebo (Lancet 2019); in the shorter AWARD-6 trial the most frequent were nausea (20%), diarrhea (12%), dyspepsia (8%), and vomiting (7%), with 6% discontinuing for adverse events. Most reports describe these effects as most pronounced early, which is the rationale behind the stepwise titration written into the approved labeling.

Is a vendor "research" dulaglutide the same thing as Trulicity? No. Trulicity is a prescription medicine with an approved label, a defined manufacturing standard, and a boxed warning about rodent thyroid C-cell tumors; material sold online as "research grade" dulaglutide carries none of those guarantees and is not the approved product. The FDA has warned specifically about unapproved and compounded GLP-1 drugs, including uncertainty about what the vial actually contains (FDA statement on unapproved GLP-1 drugs). Treating the two as interchangeable discards the entire regulatory basis for the evidence described above.
