This page summarizes safety information for elamipretide (SS-31) for educational purposes, including material from the approved Forzinity label. It is not medical advice, and it does not describe a treatment to self-administer.
There is now a label to read
Until 2025 the honest answer about SS-31 safety was "trial data only." That changed with the accelerated approval of Forzinity (elamipretide hydrochloride), which means there is a reviewed prescribing information document with named warnings, quantified adverse reactions, and specified handling (Forzinity prescribing information, DailyMed). That is a substantially better source than community reports — and it describes a supervised product used in one rare disease, not the material sold as research-grade SS-31.
The labeled warnings
Three items appear in the label's warnings and precautions:
- Benzyl alcohol toxicity. The formulation contains 20 mg of benzyl alcohol per mL. The label describes serious and fatal reactions in neonates, including metabolic acidosis progressing to neurotoxicity and, in some cases, gasping syndrome — and the product is not approved for use in neonates.
- Hypersensitivity reactions. Serious allergic reactions can occur, including reactions requiring emergency intervention.
- Eosinophilia. Named as a labeled precaution.
Two of those three would be invisible from mechanism alone. The benzyl alcohol issue is a property of the formulation rather than of the peptide, which is a good illustration of why "the molecule is well tolerated" and "the product is safe" are different claims.
Injection-site reactions, with numbers
The most common adverse reactions are local, and the label quantifies them from the pivotal trial population:
- Local administration reactions in 12 of 12 (100%) elamipretide-treated patients versus 8 of 12 (67%) on placebo.
- Injection-site erythema: 100% versus 25%.
- Injection-site pain: 75% versus 42%.
- Injection-site induration: 67% versus 17%.
- Injection-site pruritus: 67% versus 17%.
A 100% incidence is not a footnote. Daily subcutaneous administration of this peptide produces a local reaction in essentially everyone, and the trial populations were small enough that these percentages describe a handful of people each — worth keeping in mind when reading them.
The same finding recurs across the programme. Injection-site reactions were the most common adverse event in the 168-week TAZPOWER open-label extension (PMID 38602181), and in the ReCLAIM phase 1 ophthalmology studies adverse events were mild or moderate and predominantly injection-site related (PMID 36246187).
Tolerability across the trial programme
Beyond local reactions, the systemic tolerability record is reassuring within the settings studied. MMPOWER-3 randomized 218 participants to 40 mg/day subcutaneous elamipretide or placebo for 24 weeks; although it missed both co-primary endpoints, the authors concluded that subcutaneous elamipretide is well tolerated (Karaa et al., Neurology, 2023). PROGRESS-HF likewise reported elamipretide well tolerated at 4 mg and 40 mg daily over 28 days in 71 patients with heart failure (PMID 32068002).
Those populations matter. They were people with mitochondrial myopathy, heart failure, or retinal disease, enrolled under monitoring, with exclusion criteria and scheduled safety visits. The label also specifies a dose reduction in adults with severe renal impairment, which is the kind of adjustment only a supervised setting can apply.
Why the label does not cover research-grade SS-31
Forzinity is a specified 280 mg/3.5 mL (80 mg/mL) sterile solution, stored refrigerated at 2–8 °C and discarded eight days after first opening. Material sold as "SS-31" is none of those things: no verified concentration, no sterility assurance, no impurity specification, no stability data, and no labeled warnings accompanying it.
The distinction matters for safety in a concrete way. The benzyl alcohol warning applies to a formulation whose composition is disclosed; an unlabeled vial's excipients are unknown. And the hypersensitivity reactions described in the label were managed in monitored settings, some requiring emergency treatment. An approved product with named risks and a research chemical with unknown contents are not the same safety proposition, even when both contain the same peptide.
